Vaccinating on time means healthier children, families and communities.

The Florida Department of Health offers no-cost vaccines for eligible children. A small administration fee may apply.

Back to School Shots

No-cost school physical with immunizations

Through September 1, children with current immunizations may receive a free school physical at Florida Department of Health in Volusia County locations.

Child immunizations always are offered at no-cost and without an appointment. However, appointments are needed for school physicals.

Immunizations Records

Official immunizations records (DOH Form 680 "Blue Cards") are available.

Transferring students in need of an official immunization record (DOH Form 680) must provide their immunization record from their previous provider. Records may be submitted in advance by:

Providing records in advance by one of the methods will decrease wait time for receiving an official Florida record (DOH Form 680).
