The Florida Department of Health in Volusia County (DOH-Volusia) participates in a variety of health fairs and community events throughout the year. To request DOH-Volusia participation in a Health Fair/Community Event, please fill out the form below.

Once you have completed and submitted the request form, a staff member will contact you. Requests must be made at least two weeks prior to your event.
