Ryan White Program
The Ryan White HIV/AIDS Program is a comprehensive system of care designed specifically for people living with HIV. The system of care includes primary medical care and essential support services for people who are uninsured or underinsured. The Florida Department of Health in Volusia County is responsible for assessing the needs of people living with HIV/AIDS, developing a comprehensive plan to meet identified needs, prioritizing services and allocating funds in support of those services and evaluating the efficiency of the system of care.
Documents needed for Ryan White Eligibility:
- Proof of Identification photo: (supply one of the following) current driver’s license, official state ID, passport, ID from correctional facility, current military ID, US Visa.
- Client must have a verified HIV positive diagnosis. (supply one of the following) positive western blot result, medical provider diagnosis, presumptive diagnosis based upon lab results or positive outreach result, Detectable HIV viral load, or A viral resistance test result. Please also bring any medical therapies prescribed by a medical provider.
- Proof of Residency: (supply one of the following ) current driver’s license with address, official state ID that includes the current address, certificate of residence/address, rent receipts, utility bill with name and current address, or a notarized statement from the landlord, school records, homeless shelter/social services agency letter signed by staff, property tax receipt or W-2 from previous year, current voter registration card, Green card, prison record, or current documentation from Medicare/Medicaid.
- Income Qualification (Client Gross income cannot be more than 400% of the Federal Poverty Level (FPL). Documentation or proof of income is required for all clients: (supply one of the following) (Social Security award letter current year, Copy of Social Security check, Bank statements showing automatic deposit of Social Security check, Pay Stubs, year-end 1099, W2 tax form from employer, Federal Income tax return, unemployment insurance income, alimony, child support, SNAP, Supplement Security Income, WIC, Medicaid, TANF, self-employment income, Department of Labor, or a notarized letter from person providing income, food, housing).
Recertification is required every six months to include verification of income and document any changes to ensure continued Ryan White eligibility.
